1. Decide which type of membership you desire.
   
   
2. Either print, complete, and mail your application form to:

   Candler, NC 28715
   
   NOTE: don't forget to include your credit card information or a check with your application.
   
   
3. Or complete and email the application to: MedBFB@ gmail.com and pay by credit card securely and confidentially by using the IPX Services, Inc secure and strictly confidential credit card gateway.
   
   
4. Your annual benefits begin when your payment and application are received and processed.

Please Note: Effective July 1, 2009, members who fail to renew before their membership lapses will be subject to a $25 late renewal fee. Please renew promptly to avoid this!
